Just Breathe

Breath is the bridge which connects life to consciousness, which unites your body to your thoughts. Whenever your mind becomes scattered, you can use your breath as the means to take hold of your mind again. That’s Thich Nhat Hanh, and he understood something most people miss. Your breath is one of the most powerful tools you have for managing your nervous system, and you’re probably ignoring it.

Breathing is unconscious. We do it at least twelve times a minute without even noticing, adding up to six to seven million breaths a year, and for the vast majority of them we don’t need to think at all. Your body handles it automatically whilst you focus on everything else.

But here’s what’s interesting. Although breathing happens automatically, it’s also under voluntary control. You can consciously change your breathing pattern, and when you do, you directly affect your nervous system, your stress response, and your emotional state.

If you feel anxious, panicky, or scared, you’ll also unconsciously tend to breathe more quickly. Which is exactly why one of the things you can do if you feel anxious is learn to control your breathing deliberately. In this way, you can bring back a degree of balance even in high-pressure situations. This article introduces a few commonly used breathing techniques that can bring back some control when you feel anxiety creep up on you.

The Physiology of Breathing

For thousands of years, breathing has been used as a focus to help relax the whole body. Breathing is at the heart of yoga, meditation, and mindfulness practice. Research shows that breathing techniques, when practised regularly, lower stress levels, reduce pulse rate, lower blood pressure, and reduce the release of stress hormones.

This isn’t mystical. It’s mechanical. Your breathing directly affects your autonomic nervous system. When you breathe rapidly and shallowly — chest breathing — you activate your sympathetic nervous system, fight or flight mode. When you breathe slowly and deeply — diaphragmatic breathing — you activate your parasympathetic nervous system, rest and digest mode.

Studies using heart rate variability as a measure of nervous system activity show this clearly. Slow, controlled breathing increases heart rate variability, indicating increased parasympathetic tone. Fast, shallow breathing decreases it, indicating sympathetic dominance. You can literally shift your nervous system state by changing how you breathe. Your breath is a bridge between conscious and unconscious control — a lever you can pull to shift your entire physiological state.

Diaphragmatic Breathing

At the heart of yoga is pranayama, the formal practice of controlling the breath. One well-known form is the three-part breath, which teaches you to breathe with your diaphragm, filling the lower part of your lungs first before your upper chest: draw a breath into your lower belly, then raise the lower rib cage, then continue to breathe into the upper chest.

By practising this type of breathing, you interrupt the fight-or-flight response and encourage a relaxation response in your body. Filling the lower lungs stimulates the vagus nerve, which runs from your brainstem through your chest and abdomen and is the primary pathway of your parasympathetic nervous system. When you stimulate it through diaphragmatic breathing, you’re directly telling your body to relax.

At all times, breathe through your nose. Try to match the time taken to breathe in with the time taken to breathe out, at around five seconds for each. Six breaths a minute like this is close to what’s called the resonance frequency, the rate at which your heart rate and blood pressure naturally fall into phase, producing the largest possible increase in heart rate variability — it’s not an arbitrary number, it’s close to the physiological optimum (Lehrer, Vaschillo & Vaschillo, 2000).

Some yoga traditions add a gentle constriction at the back of the throat as you breathe, called Ujjayi or Ocean Breath, because the sound it makes resembles rhythmical waves. Other people call the belly-first version simply belly breathing.

The main thing about this type of breathing is that you’re getting your belly to fill first. This is the key because it triggers the parasympathetic, relaxation part of your nervous system. Research on vagal tone shows that regular diaphragmatic breathing practice increases baseline vagal activity, meaning you become more resistant to stress over time. Your nervous system becomes more resilient, and you don’t react as strongly to stressors because your baseline state is already more relaxed.

Box Breathing

Box breathing, also called square breathing, uses a regular four-part pattern: breathe in for four seconds, hold for four seconds, breathe out for four seconds, hold for four seconds, then repeat. Used effectively, this can help avoid panicking in stressful situations by giving your brain a controlled, repeatable pattern to follow.

The technique takes some practice, and the holds can feel longer than comfortable, especially if you’re already breathing rapidly. That’s fine — focusing on your breathing is far better than focusing on whatever else is stressing you out. This technique is often associated with military and elite performance training, and whatever its origin, the underlying physiology holds up on its own: when you control your breath, you influence your nervous system, and when you influence your nervous system, you influence your response to stress.

A note of caution: breath-holding isn’t right for everyone. If you have uncontrolled high blood pressure, significant heart or lung disease, are pregnant, or are prone to fainting, take this one more gently, or check with your GP first.

Mindfulness and Breathing

Mindfulness is a conscious state of focusing your awareness on the present moment whilst also acknowledging your feelings, thoughts, and bodily sensations. To get into a mindful state, there’s nothing better than using your breath as a tool to do this.

Bring all your attention to your breathing: feel your breath enter your body, notice where you feel it, and notice how each breath feels different from the last. Find the changeover point between your in-breath and out-breath. These are little things you can use in a guided or self-guided mindfulness session to bring your consciousness back into the present. Free resources, including the NHS’s own mindfulness materials and several apps with free tiers, are a good place to start.

Research on mindfulness-based interventions shows that breath-focused meditation is associated with measurable changes in brain activity within eight weeks of regular practice: the Default Mode Network becomes less active during rest, and the prefrontal cortex, responsible for executive function and emotional regulation, becomes more active.

Power Breathing

You don’t need to ask an athlete or bodybuilder how important the right breathing technique is before exertion. What probably happens, without you thinking about it, is that you take a few deep breaths in preparation, hold for a moment while you brace your core, and put your effort into whatever’s about to happen.

It’s not just lifting logs or straining at weights. Breathing is an essential part of doing anything that requires effort, and when we’re nervous or anxious, we sometimes forget to breathe, which only causes more anxiety. A few big, deep belly breaths before something strenuous or demanding — a presentation, a difficult conversation, a job interview — prepares you physically and mentally. Studies on performance psychology show that people who use pre-performance breathing routines report lower anxiety, better focus, and improved performance. It works.

Why Breathing Works When You’re Panicking

When you’re panicking, your breathing becomes rapid and shallow, part of the fight-or-flight response as your body tries to increase oxygen availability for emergency action. But rapid, shallow breathing actually makes panic worse: over-breathing causes you to exhale too much carbon dioxide, which alters your blood chemistry and triggers additional symptoms — tingling in your extremities, light-headedness, the frightening sensation of not being able to get enough air, even though you’re breathing too much. These symptoms can make you think you’re having a heart attack or dying, which makes you panic more, which makes you breathe faster, which makes the symptoms worse. It’s a vicious cycle.

The way to break it is counterintuitive, and it’s worth getting right: during panic, the goal is to breathe more slowly, and if anything a little more shallowly, rather than more deeply. What matters most is the rate, not the depth — the aim is to let your carbon dioxide levels come back up, not to pull in more air (Meuret et al., 2008). This is also why the old advice to breathe into a paper bag worked, and why the sensation of ‘I can’t get enough air’ during a panic attack is misleading you about what your body actually needs.

Research on panic disorder shows that teaching people controlled breathing techniques significantly reduces panic attack frequency and severity — not because breathing cures the underlying anxiety, but because it interrupts the physiological panic cycle before it escalates into a full attack.

I think of a patient, a composite of several people I’ve seen with this pattern, who used to get panic attacks on the Underground. The moment the train entered a tunnel, she’d start hyperventilating and quickly become convinced she was dying, having to get off at the next station on multiple occasions. Alongside some talking therapy, we worked on box breathing: every journey, the moment she felt anxiety rising, she’d start the four-by-four pattern. Within three weeks, she could ride the whole line without incident. The anxiety was still there, but the breathing prevented it from escalating into full-blown panic.

The Daily Practice

Here’s what most people get wrong about breathing techniques. They wait until they’re panicking to use them. That’s like waiting until you’re drowning to learn to swim. It works far better if you practise beforehand.

Five minutes daily is enough. Pick a technique — diaphragmatic breathing is a good default — set a timer, and practise. Do it when you’re calm so it becomes automatic. Then, when stress hits, your body already knows what to do.

Why You’re Not Already Doing This

You’ve probably heard about breathing techniques before. Everyone has. So why aren’t you using them? Because they seem too simple, too easy. Surely something as basic as breathing can’t actually affect your stress levels, your panic attacks, your anxiety.

Except it can, and the research is substantial. Controlled breathing reduces stress, lowers blood pressure, decreases panic, improves mood, enhances performance, and gives you immediate physiological control over your stress response.

The problem isn’t that breathing doesn’t work. The problem is that it’s so simple that people assume it can’t be powerful. They’re waiting for something complex, something difficult, something that requires specialised equipment or training. You don’t need any of that. You just need to breathe. Properly. Deliberately. Regularly.

If panic attacks are a regular feature of your life, please know that they respond very well to treatment, and you don’t need to manage them alone — your GP or NHS Talking Therapies, which you can self-refer to, are both reasonable places to start.

You don’t need to be a yoga instructor to realise how important breathing is when you exercise, and you don’t need to practise mindfulness to realise how beneficial it is for your mind. In short, just remember to breathe.
